It was Superboy-Prime, when trying to break the realities barrier. His punches caused many abnormalities, like the Doom Patrol being ressurected (or somethin' like that), and one of these things was Jason Todd coming back to life

Thankfully there was Crisis on Infinite Earths, which destroyed all of those parallel universes. The sole survivors from this mess that weren't incorporated into Earth-1 were Superboy-Prime (from Earth Prime, were the only super-hero was Superboy), Earth-2 Superman and Lois Lane (Golden Age universe), and Earth-3 Alexander Luthor (anti-matter universe, everyone is a villains, except Lex Luthor and his son, Alexander Luthor).
In Infinite Crisis they explained some continuity errors ("This doesn't make sense? Blame Superboy-Prime!") and fixed others. Jason Todd was a bonus from someone who drank too much while writing the argument.... But I liked seeing him causing havoc. The bat-boys are going nuts

I think it's not hard to find Crisis on Infinite Earths to buy, even at DC direct. It's when Wally West becomes the Flash (and Barry dies) and Guy Gardner gets his ring
As for ressurections, Superman got a very good regeneration chamber plus his own kryptonian genetics helped him with this (sun helps him, so...). Green Arrow was ressurected by Parallax (Hal becomes Parallax in Esmerald Twilight/New Dawn after the events of the Return of Superman (Reign of Supermen)) during the Zero Hour event (shown later in Green Arrow series).... Hal Jordan was reborn in Green Lantern Rebirth... And, of course, there is Jason Todd. The rest, they now usually stay dead for good.
